A 55-year-old man arrived in the emergency department having been found collapsed in his kitchen 6 hours previously. He was last seen well by his wife an hour before he was found. On arrival in the emergency department, he was alert and eye-opening spontaneously, and obeying commands with his left hand (he was unable to move his right side). He was attempting to speak but was unable to do so. His pulse rate was 126bpm and the rhythm was atrial fibrillation. His blood pressure was 203/110mmHg.
